Health Form Info

The health form may look scary, but it’s just so we can take care of you a little better!

There are three good reasons for having these health forms:

  1. It lets us know about medical conditions or allergies you might have.
  2. It gives us emergency contact numbers, so we can call your parent or guardian if there was an emergency.
  3. It lets us know you have your parent/guardian’s permission to be at One80 each week.

Regular attendees of One80 are asked to fill out one of these permission and health forms each year to help us better care for you. They are kept in compliance with NSW Privacy and Confidentiality laws and the information contained won’t be distributed to other parties.
